Name Change Documentation for Academic Records Verification

To earn an Imaging Assistant (I.A.) credential, you must be at least 18 years old and have a high school diploma or GED. Once you create an account and start earning this credential, you'll upload your academic records in the I.A. documentation tool. 

If your name has changed from what’s on your academic records, you'll also need to upload legal documentation, such as marriage certificates, divorce decrees, or court orders. You’ll need to show ALL name changes that have occurred between the name on your transcript and your current name. Additionally, if any supporting document is not written in English, it must be accompanied by a signed English translation document.

  • The marriage certificate must include the date of the marriage and the signature of the person who performed the ceremony. If no third-party officiant performed the ceremony, the marriage certificate must show that. 
  • Your name on the marriage certificate must match your current name on record with ARRT. 
    • If it doesn’t match, you’ll also need to provide legal documentation showing how your name changed from what appears on ARRT records to the name on the marriage certificate. 
  • You don't need to upload your entire divorce decree. Instead, submit documentation that provides all three of these required items: 
    1. Your name as it currently appears in your ARRT record (typically shown on the first page of a divorce document) 
    2. A section stating the resumption of a former name, and
    3. A judge’s signature or a filed stamp from the Clerk of Court 
  • If the document doesn’t stipulate a name change, you must also provide two of the following that show your new legal name:
    • A signed Social Security card
    • A valid driver’s license
    • A passport
  • The documents must contain:
    • Your name before the change went into effect
    • A section stating what the name was changed to
    • A signature of a court official
  • Birth certificate, passport, and driver's license are accepted only when individuals are:
    • Changing their abbreviated first name (e.g., Kathy to Katherine)
    • Adding a middle name shown on the document, or changing their middle name to their middle initial
    • Proving the correct spelling of their name
    • Adding or removing a name suffix (e.g., Jr., Sr.)
  • The Petition for Name Change alone is not a standalone document. It must be accompanied by the Certificate of Naturalization. 
  • The Certification of Naturalization must be signed with your new name.
